Rebel Wellness Membership Terms & Conditions

1. Billing
A valid card must remain on file. Memberships are billed automatically on the 1st of each month. Payments are non-refundable and non-transferable. Rebel Wellness may adjust pricing with notice.
2. Commitment & Early Cancellation
Memberships require a 3-month (90-day) minimum commitment. This ensures fairness, consistent scheduling, and integrity of member pricing. If canceled before 90 days, a $125 early termination and administrative fee applies to cover lost revenue and administrative costs. The fee will be charged to the card on file or invoiced if payment fails. Unpaid invoices are due within 10 business days.
3. Introductory Period Restriction
Membership pricing activates after the first billing cycle. Clients who cancel before completing 90 days will be charged the difference between the member and standard session rate for any sessions used, in addition to the $125 fee.
4. Session Use & Rollover
Each membership includes one session per month based on tier. One unused session may roll over and must be used within 3 months while active. Unused sessions are forfeited when the membership ends or becomes inactive.
5. Cancellations & No-Shows
Appointments must be canceled or rescheduled at least 24 hours in advance. Late cancellations or no-shows result in a lost session or a 50% fee if no balance remains. Repeated issues may result in termination.
6. Pause Policy
Memberships may be paused up to 3 months per year. During a pause, sessions and member pricing are inactive. Accrued sessions remain valid within the 3-month rollover window when resumed.
7. Inactivity & Rejoin Waitlist
If inactive for 30 consecutive days (no sessions and unpaid), the membership will be canceled and all unused sessions forfeited. Members who cancel early must wait 6 months before re-enrolling to protect fairness and scheduling access.
8. Refunds & Conduct
No refunds for partial months, unused sessions, or early cancellations. Rebel Wellness is a trauma-informed, professional space. Inappropriate behavior results in immediate dismissal and termination of membership privileges.

By enrolling, you confirm that you have read, understood, and agree to these terms. Enrollment and participation serve as your digital signature of consent and agreement.

In accordance with HIPAA regulations and the scope of massage therapy practice, we ensure that all client information is protected and handled with strict confidentiality. Your health history, session details, and personal information are safeguarded, and only shared with your explicit consent or as required by law. We value your trust and prioritize your privacy at every step of your wellness journey.
