Monthly Membership Terms & Conditions

Welcome to Rebel Wellness. Monthly Memberships are designed to provide consistent, integrative massage care for clients who value ongoing support rather than occasional sessions. Please review the following terms carefully before enrolling.

1. Billing & Payment Authorization

A valid credit or debit card must remain on file for all memberships.

Monthly Memberships are billed automatically on the 1st of each month. By enrolling, you authorize Rebel Wellness to charge the card on file for recurring membership payments.

All payments are non-refundable and non-transferable. Failed or declined payments may result in temporary suspension or cancellation of membership privileges until resolved.

2. Membership Activation Period & Commitment

All Monthly Memberships require a 90-day minimum activation period, beginning on the date of enrollment.

By enrolling, you agree that your membership must remain active for at least 90 consecutive days, regardless of session usage. Memberships cannot be canceled during this activation period.

This policy exists to:

• Support consistency of care

• Protect reserved scheduling availability

• Prevent misuse of membership pricing

3. What’s Included in a Monthly Membership

Each Monthly Membership includes one integrative massage session per month at the selected duration.

Sessions may include, as appropriate:

• Integrative massage therapy techniques

• Hot towels

• Targeted hot and/or cold stone spot treatment

• Acupressure techniques, including use of tools such as an acupressure pen

Techniques are selected based on session goals and professional judgment. Not all tools or techniques are used during every session.

4. Session Use & Carryover

Memberships include one session per billing month.

Sessions are intended to be used within the billing month. One unused session may carry over for up to 30 days in limited circumstances such as illness, emergency, or scheduling conflicts.

Carryover:

• Is not automatic

• Must be approved by Rebel Wellness

• Does not extend beyond 30 days

Unused sessions beyond this window are forfeited. Sessions do not accumulate indefinitely.

Membership sessions are for the enrolled client only and are non-transferable.

5. Appointment Cancellation & No-Show Policy

A 48-hour notice is required to cancel or reschedule a membership appointment.

Appointments canceled with less than 48 hours’ notice, no-shows, or late arrivals that prevent completion of the session will result in the session being forfeited for that month.

This policy protects reserved time and ensures fairness to all clients.

6. Membership Cancellation After Activation

After the 90-day activation period has been completed, memberships may be canceled with a minimum of 30 days’ written notice prior to the next billing date.

Cancellation requests received after billing has processed will apply to the following month.

7. Pause Policy

Temporary pauses may be considered on a case-by-case basis for medical or significant life circumstances.

• Pauses are not guaranteed

• Sessions cannot be booked or used during a pause

• Approved pauses do not extend session carryover beyond the stated 30-day policy

All pause requests must be submitted directly to Rebel Wellness for review.

8. Add-On Services & Enhancements

Add-on services are not included in Monthly Membership pricing and carry an additional fee.

This includes, but is not limited to:

• CBD products or enhancements

• Specialty oils

• Extended-time services

• Any add-on requiring additional materials or preparation

Add-on pricing is subject to change.

9. Legacy Memberships

Clients enrolled in legacy or previously established membership plans retain the pricing and terms agreed upon at the time of enrollment, provided their membership remains active.

Legacy plans are not publicly listed and are not available for new enrollment.

10. Refund Policy

All membership payments are non-refundable.

No refunds will be issued for unused sessions, missed appointments, partial months, pauses, or early termination.

11. Conduct, Termination & Policy Modifications

Rebel Wellness reserves the right to update membership terms with notice.

Memberships may be terminated if policies are violated or conduct is inappropriate. Upon termination or cancellation, any unused sessions are forfeited.

Agreement

By enrolling in a Monthly Membership, you acknowledge that you have read, understood, and agree to all terms outlined above. Enrollment and participation serve as your digital consent to this agreement.
