This service is exclusively available to Subscribed Sacred Rebel Members with an active monthly massage membership. Membership ensures access to this offering, along with the benefits and consistency that come from being part of our Rebel Wellness community.
Cancellation Policy
Rebel Wellness Membership Terms & Conditions Effective: October 2025 Location: 116 5th Street, West Des Moines, IA Contact: tammy.rebelwellness@gmail.com 1. Billing A valid card must remain on file. Memberships are billed automatically on the 1st of each month. Payments are non-refundable and non-transferable. Rebel Wellness may adjust pricing with notice to reflect operational or economic changes. 2. Session Use & Rollover Each membership includes one all-inclusive session per month, based on your tier. One unused session may roll over at a time and remains valid for up to 3 months while your membership is active. Unused sessions are forfeited if membership is canceled or terminated. 3. Scheduling & Cancellations Please cancel or reschedule at least 24 hours before your appointment. Late cancellations or no-shows will result in the session being deducted or charged at 50% of the session rate if no balance remains. Members are encouraged to schedule early to secure preferred times. 4. Commitment & Cancellation A 3-month minimum commitment is required. After that, memberships continue month-to-month. Cancellations require 30 days’ written notice by email or in person, submitted prior to the next billing cycle to avoid additional charges. 5. Pause Policy Memberships may be paused up to 3 months per calendar year. During a pause, sessions and member pricing are inactive. Accrued sessions remain valid within the 3-month rollover limit once resumed. 6. Inactivity & Legacy Status Legacy status applies only to active members. If inactive for 30 consecutive days (no sessions attended and payment not current), legacy status is forfeited. You may rejoin at the current published rates. 7. Termination Rebel Wellness may terminate memberships for repeated late cancellations, nonpayment, policy violations, or inappropriate conduct. All unused sessions are forfeited upon termination. 8. Conduct & Liability Rebel Wellness is a trauma-informed, professional space. Inappropriate behavior results in immediate dismissal and loss of membership. Massage and wellness services are not a substitute for medical care. By joining, you release Rebel Wellness from liability for injury or loss except in cases of gross negligence. Agreement By enrolling, you confirm that you have read, understood, and agree to all terms above. Your enrollment and continued participation serve as your digital signature of consent.
