This is ongoing support for your nervous system, your muscles, and your overall well-being. With a minimum 3-month commitment, this membership creates structure and accountability, helping your body stay regulated instead of cycling through tension and burnout. Each month, you receive a 90-minute Integrative Massage fully customized to your body, plus three add-ons of your choice to enhance your session and deepen your results. At Rebel Wellness in West Des Moines, this level of care allows more time to work through chronic tension, support recovery, and address both physical and nervous system patterns more effectively. Cancellation Policy
Rebel Wellness Membership Terms & Conditions Effective: October 2025 Location: 116 5th Street, Historic Valley Junction, West Des Moines, IA Contact: tammy.rebelwellness@gmail.com 1. Billing A valid card must remain on file. Memberships are billed automatically on the 1st of each month. Payments are non-refundable and non-transferable. Rebel Wellness may adjust pricing with notice. 2. Commitment & Early Cancellation Memberships require a 3-month (90-day) minimum commitment. This ensures fair access to appointment times and supports the membership structure. If canceled before the 90-day term ends, a $125 early termination and administrative processing fee applies to cover lost revenue and administrative costs. This fee will be charged to the card on file or invoiced if payment fails. Unpaid invoices are due within 10 business days. 3. Session Use & Rollover Each membership includes one all-inclusive session per month based on tier. One unused session may roll over and must be used within 3 months while the membership remains active. Unused sessions are forfeited when the membership ends or becomes inactive. 4. Cancellations & No-Shows Appointments must be canceled or rescheduled at least 24 hours in advance. Late cancellations or no-shows forfeit that session or incur a 50% fee if no balance remains. Frequent no-shows may result in membership termination. 5. Pause Policy Memberships may be paused up to 3 months per calendar year. During a pause, sessions and member pricing are inactive. Accrued sessions remain valid within the 3-month rollover period once resumed. 6. Inactivity & Termination If inactive for 30 consecutive days (no sessions and payment not current), the membership will be canceled and all unused sessions forfeited. Rebel Wellness may terminate memberships for nonpayment, repeated cancellations, or inappropriate conduct. 7. Refunds & Credits No refunds are issued for partial months, unused sessions, or early cancellations. Membership fees cannot be converted into cash or credit. 8. Conduct & Professionalism Rebel Wellness is a trauma-informed, professional practice. Any inappropriate behavior results in immediate dismissal and loss of membership privileges. Agreement By enrolling, you acknowledge that you have read, understood, and agree to these terms. Your enrollment and continued participation serve as your digital signature of consent and agreement.
